Forms for funded EHAP 16 agencies
All forms should be forwarded with the required information to the Designated Local Board (DLB) by mail. The Department of Housing and Community Development (HCD) requires that original signatures be submitted to the DLB. No faxed/emailed copies will be accepted. The address is:
